Russia - New Patient Admission System Saves City Polyclinics from Queues

Russia (bbabo.net), - The incidence of coronavirus in the capital is decreasing this week. It's been at the same level for several days now. "I hope that we are reaching a plateau," Moscow Mayor Sergei Sobyanin said today, February 2, at a meeting of the presidium of the government's coordinating council to combat coronavirus.

The arrival of the omicron strain and the speed of its spread forced the city authorities to look for additional opportunities to provide Muscovites with medical services. Now citizens can send an application to the official portal of the mayor mos.ru and with its help not only get an audio consultation of a doctor, but also without leaving home, apply for a sick leave, and after recovery, close it. Andrey Tyazhelnikov, the chief physician of the consulting and diagnostic polyclinic No. 121, shared with the correspondent how it works.

"Our clinic - one of the largest in Moscow - consists of eight branches and a head unit. We serve 290,000 residents of Northern and Southern Butovo - districts, each of which is a small city," he said. "Doctors are now receiving from 8 :00 to 20:00 seven days a week The workload is huge, and I can say without exaggeration that all the staff - not only doctors, but also nurses, administrators who do everything so that not a single patient is left without help - are just real heroes. I don’t know how we would have survived all this if it weren’t for the advent of remote reception of patients.”

Largely due to the emergence of a new service, even those patients who prefer to come to the polyclinic themselves get an appointment with the general practitioner on duty without any problems. As in the entire outpatient network of Moscow in KDP No. 121, patients with signs of SARS and covid are now being admitted in a department specially created for them: with a separate entrance, testing areas, and a doctor's examination. Therefore, they enter the doctor’s office with the result of an express test for covid and a PCR test, as well as a measured temperature. As a result, the doctor needs much less time to make a diagnosis and determine the tactics of treatment. And the patient spends no more than half an hour for the entire visit.

The same citizens who have applied to mos.ru are called by the doctors themselves. They clarify the symptoms of the disease, prescribe treatment, if necessary, also remotely open the sick leave, and then close it. Luckily, omicron is still running much easier than previous strains. He has the same symptoms as seasonal acute respiratory infections - a runny nose, sore throat, fever, weakness ... Yesterday, February 1, for example, in Moscow, 1.6 thousand patients needed hospitalization per day - that is, 6.8 percent of all those infected with the coronavirus.

The question involuntarily arises: will the doctor be able to correctly assess the patient's well-being only during a telephone conversation, without seeing the patient, without any tests and other diagnostics? Andrey Tyazhelnikov dispels doubts: “The doctor does not just talk with a person, but enters his electronic medical record through the EMIAS system. This gives him the opportunity to more accurately assess how certain symptoms of SARS or covid can affect his condition, taking into account age, existing chronic If he sees that someone needs a more complete picture, he will refer the patient to an appointment in a polyclinic or even immediately send a doctor to his home.

The new system, Andrey Tyazhelnikov is sure, on the one hand, creates a healthier epidemiological situation in clinics, since a large number of infected people do not accumulate in a medical institution. On the other hand, it gives Muscovites confidence that even if they get sick, they will not be left without qualified medical care, no matter how the pandemic develops.

Now citizens can apply for a sick leave in one of three ways: during an appointment at a polyclinic, by leaving an application for an audio consultation on mos.ru, or by calling a doctor home (in case of limited mobility or a severe course of the disease). For those who fall ill with SARS, it opens for seven days, and for patients with confirmed COVID19 - for 14 days. Moreover, the sick leave will also be opened for those who have been in contact with a covid patient, even if they do not have symptoms of SARS, but in this case, the fact of contact must be confirmed by the sick person. For those who opened the sick leave remotely, it will also be closed remotely, without visiting the clinic. It will be sent to the employer automatically. For the patient himself, the entire process of his treatment, including with the help of audio consultations, and being on sick leave, is reflected in the electronic medical record, which means that the sick leave itself cannot be lost.
